Parent/Guardian Verification & Consent

I confirm I am a parent or legal guardian of the child being registered and consent to the terms and policies of Warner Pacific University’s Esports & STEM Program. I understand that by completing this registration form for my child to participate in Warner Pacific University’s Esports & STEM Program, I am hereby authorizing and providing verifiable parental consent for Warner Pacific University and its partners to collect and retain personal information on behalf of my child for the purpose of participating in Warner Pacific University’s programs. Additionally, the Children’s Online Privacy Protection Act (“COPPA”) require us to inform parents and legal guardians about how we collect, use, and secure personal information for children.

Gameplay Authorization 

I have verified and approved the following video game titles, (i) Super Smash Bros., (ii) Fall Guys, (iii) Minecraft, (iv) League of Legends, (v) Roblox, and (vi) Rocket League, that my child will be playing during Warner Pacific University’s Esports & STEM Program. I understand that the contents and ratings of these video game titles may be subject to change and that my authorization is being given to Warner Pacific University and its partners unless I provide notice stating otherwise, sent in writing to Warner Pacific University's Esports Director. Parents and legal guardians are encouraged to review current content ratings for all video game titles being used by Warner Pacific University’s Esports & STEM Program at https://www.esrb.org/ and/or https://pegi.info/.

Player Conduct

I, and my player, acknowledge and agree to behave with good conduct at all times. Any action or behavior that is deemed to be a violation of Warner Pacific University’s Policies and/or Rules may result in disciplinary action or removal from all current and future programs. I understand that this includes behavior such as (i) inappropriate use of computers, including but not limited to inappropriate usernames, language, hate speech, or NSFW content (ii), abuse of hardware, installing applications without consent, and/or use of cheats, hacks, or abuse of in-game mechanics (iii) harassment to others physically, verbally, or through in game-mechanics such as excessive emoting and taunting, (iv) text/voice spamming, and (v) any conduct that prevents others from learning or participating.

Media Release

I understand that by completing this registration form and signing below, that pictures and/or videography may be taken of my student throughout the course of the program, including student created artworks. I further understand and authorize Warner Pacific University and its partners to collect this media which may be used in program displays, community outreach, educational materials, public marketing, and/or news purposes. I confirm that my authorization and release for this media is being given unless I provide notice stating otherwise, sent in writing to Warner Pacific University's Esports Director.

The information provided in connection with Warner Pacific University's Esports & STEM program is being provided to Warner Pacific University, a 501(c)(3) non-profit organization.  Any fees and/or dues that may be associated with Warner Pacific University’s Esports & STEM Programs are to support the environments, venues, activities, and related expenses necessary to provide safe esports and structured gaming programs for youth.
